A recent University of Edinburgh study examined how AI scribes and ambient AI tools transcribe clinical encounters. The research found these systems reliably capture spoken language but often omit nonverbal cues: facial expressions, pauses, tone, and gesture. Those signals carry emotional context and indicate distress, uncertainty, or reluctance to disclose sensitive information. When clinical notes reduce consultations to quoted facts, the patient voice can be muted and contextual details lost, which may affect diagnosis and follow-up care.
The Dual Impact: Cognitive Shifts for Clinicians
AI note-taking promises administrative relief, faster documentation, and more time with patients. But the study warns of cognitive offloading risks. Relying heavily on automated notes can weaken memory recall and clinical reasoning over time, and may reduce opportunities for trainees to develop note-taking and synthesis skills. For practicing clinicians there is a trade-off between regained time and potential deskilling in clinical judgement and narrative capture.
Designing for Trust: Preserving the Human Element in AI Healthcare
The study calls for human-centric AI that preserves patient voice rather than overwriting it. Practical recommendations include multimodal capture that records nonverbal signals, clinician-in-the-loop review workflows, metadata for confidence and uncertainty, and consent processes that inform patients about what is recorded. Longitudinal evaluation is needed to monitor effects on data integrity and clinician competence, and to measure whether patients alter disclosure when AI is present.
